How to Apply :
- The Columbia Kids Club membership is given free of charge to all children born in Columbia Asia Indonesia Hospitals, upon completion of the registration form.
- If a child is interested to join this club but is neither a sibling of a Columbia Kids Club member, nor born at Columbia Asia Hospitals, IDR 50.000 registration fee applies.

Term & Conditions :
- This card is valid from birth or registration date, until the child turns 7 (seven) years old.
- Mascot Columbia Asia Kids (Zoey) shall be given once the child turns 1 year old.
- Upon completion of the registration form, new member shall receive the same benefit as member Columbia Kids Club which children born in Columbia Asia Hospitals.
