This is due to changes in hormonal levels during pregnancy, i.e., increased estrogen level. Kidney flares tend to pre-dominate while musculoskeletal flares seem to decrease. Prompt treatment will help to keep the mother healthy.<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-302ad3d elementor-widget elementor-widget-spacer\" data-id=\"302ad3d\" data-element_type=\"widget\" data-widget_type=\"spacer.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<div class=\"elementor-spacer\">\n\t\t\t<div class=\"elementor-spacer-inner\"><\/div>\n\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e51e581 elementor-widget elementor-widget-heading\" data-id=\"e51e581\" data-element_type=\"widget\" id=\"subtopic03\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Some points to note:<\/h3>\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-a16968e elementor-widget elementor-widget-text-editor\" data-id=\"a16968e\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<ul><li>Women with SLE have a higher rate of miscarriage and premature births compared with the general population.<br \/>\u00a0<\/li><li>Women who have anti-phospholipid antibodies are at a greater risk of miscarriage in the second trimester because of their increased risk of blood clotting in the placenta.<br \/>\u00a0<\/li><li>SLE patients with a history of kidney disease have a higher risk of pre-eclampsia (hypertension with a build-up of excess watery fluid in cells or tissues of the body).<br \/>\u00a0<\/li><li>Pregnant women with SLE, especially those taking corticosteroids, are also more likely to develop high blood pressure, diabetes, hyperglycemia (high blood sugar), and kidney complications, so regular care and good nutrition during pregnancy are essential.<br \/>\u00a0<\/li><li>It is advisable to have access to a neonatal (newborn) intensive care unit at the time of delivery in case the baby requires special medical attention<\/li><\/ul><p><br \/><br \/><strong>Monitoring during pregnancy<\/strong>\u00a0In women with stable kidney disease before the pregnancy, the pregnancy does not jeopardize kidney function in the long term. However, SLE kidney disorder may manifest for the first time in pregnancy. Doctors must take note that lupus flares may be difficult to diagnose during pregnancy since many of the features may also occur in normal pregnancy.<br \/><br \/>Monthly follow-ups are best done in combined (obstetric and medical) clinics. History and physical examination remains the mainstay of monitoring disease activity in pregnancy. Early control of disease activity is essential. Initial assessment involves:<br \/>\u00a0<\/p><ul><li>testing for anti-ro and anti-la antibodies<br \/>\u00a0<\/li><li>testing for anti-phospholipid antibodies for those with past pregnancy losses.<\/li><\/ul>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-db9dd33 elementor-widget elementor-widget-spacer\" data-id=\"db9dd33\" data-element_type=\"widget\" data-widget_type=\"spacer.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<div class=\"elementor-spacer\">\n\t\t\t<div class=\"elementor-spacer-inner\"><\/div>\n\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-509a125 elementor-widget elementor-widget-heading\" data-id=\"509a125\" data-element_type=\"widget\" id=\"subtopic04\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Management of lupus pregnancy<\/h3>\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-c02fec9 elementor-widget elementor-widget-text-editor\" data-id=\"c02fec9\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<p>Medications which can be used during pregnancy are:<br \/>\u00a0<\/p><ul><li>aspirin or non-steroidal anti-inflammatory drugs (NSAIDs) which are used in the relief of joint or muscle pain. NSAIDs other than aspirin should be avoided in the second and third trimester.<br \/>\u00a0<\/li><li>anti-malarials such as plaquenil which is useful for those with skin and joint disease.<br \/>\u00a0<\/li><li>low-dose prednisolone. As pregnancies should be planned only when the disease is under control, most patients are already be on low-dose prednisolone by the time they become pregnant. As there are certain risks of gestational diabetes and hypertension, doctors should try to keep the prednisolone below the 10mg daily level.<\/li><\/ul><p><br \/><br \/>In the event of a lupus flare, intravenous methylprednisolone can be used. However, cyclophosphamide should be avoided because of its association with birth defects.<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-54815d8 elementor-widget elementor-widget-spacer\" data-id=\"54815d8\" data-element_type=\"widget\" data-widget_type=\"spacer.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<div class=\"elementor-spacer\">\n\t\t\t<div class=\"elementor-spacer-inner\"><\/div>\n\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-cbc1c4b elementor-widget elementor-widget-heading\" data-id=\"cbc1c4b\" data-element_type=\"widget\" id=\"subtopic05\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Decision to terminate the pregnancy<\/h3>\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-626d13f elementor-widget elementor-widget-text-editor\" data-id=\"626d13f\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<p>Doctors take into consideration the following factors:<br \/>\u00a0<\/p><ul><li>is the mother\u2019s health likely to deteriorate irreversibly if pregnancy continues?<br \/>\u00a0<\/li><li>is the baby likely to grow better delivered than in the womb?<\/li><\/ul><p><br \/><br \/>Maternal complications are more often pregnancy related than lupus related. Vaginal or caesarian section is an obstetric decision. Specific lupus issues rarely determine the delivery route, though they may upgrade urgency for operation.<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-46aef9d elementor-widget elementor-widget-spacer\" data-id=\"46aef9d\" data-element_type=\"widget\" data-widget_type=\"spacer.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<div class=\"elementor-spacer\">\n\t\t\t<div class=\"elementor-spacer-inner\"><\/div>\n\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-972ccd6 elementor-widget elementor-widget-spacer\" data-id=\"972ccd6\" data-element_type=\"widget\" data-widget_type=\"spacer.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<div class=\"elementor-spacer\">\n\t\t\t<div class=\"elementor-spacer-inner\"><\/div>\n\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-3d569dd elementor-widget elementor-widget-heading\" data-id=\"3d569dd\" data-element_type=\"widget\" id=\"subtopic06\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Post-partum period<\/h3>\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-6e41818 elementor-widget elementor-widget-text-editor\" data-id=\"6e41818\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<p>it is advisable to continue steroid medication for two to three months after the post-partum period (six weeks after delivery) if there was active SLE throughout the pregnancy. Care should be taken as flares are most frequent in the second and third trimester and the post-partum period. It would be useful to alert the neonatology staff about the new mother\u2019s anti-ro and anti-la status.<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-519bf00 elementor-widget elementor-widget-spacer\" data-id=\"519bf00\" data-element_type=\"widget\" data-widget_type=\"spacer.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<div class=\"elementor-spacer\">\n\t\t\t<div class=\"elementor-spacer-inner\"><\/div>\n\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-6231ccb elementor-widget elementor-widget-heading\" data-id=\"6231ccb\" data-element_type=\"widget\" id=\"subtopic07\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Breast feeding<\/h3>\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-ae2d273 elementor-widget elementor-widget-text-editor\" data-id=\"ae2d273\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<p>SLE patients can breastfeed. Generally very little steroid is transmitted to breast milk, especially when the patient is on low-dose prednisolone. The neonatal effect of using 20mg prednisolone daily or less is insignificant. The exposure of the baby to steroids can be further minimized by breastfeeding just prior to taking the prednisolone. Anti-coagulants such as heparin are safe, while cytotoxics such as cyclophosphamide are not safe.<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-8c66608 elementor-widget elementor-widget-spacer\" data-id=\"8c66608\" data-element_type=\"widget\" data-widget_type=\"spacer.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<div class=\"elementor-spacer\">\n\t\t\t<div class=\"elementor-spacer-inner\"><\/div>\n\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-6b1c1a9 elementor-widget elementor-widget-heading\" data-id=\"6b1c1a9\" data-element_type=\"widget\" id=\"subtopic08\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Neonatal lupus<\/h3>\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-a28c068 elementor-widget elementor-widget-text-editor\" data-id=\"a28c068\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<p>This is a rare disorder that can occur in newborn babies in which auto-antibodies in the mother\u2019s blood called anti-ro and anti-la are thought to have the potential effect of skin rashes and to injure fetal heart conduction tissue. There are usually skin rashes during the first to third months after birth, which disappear without treatment after about six months. In those rare cases (less than 2%) where there is congenital heart block, the condition is usually irreversible and the baby may require a pacemaker.<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-6eb6854 elementor-widget elementor-widget-spacer\" data-id=\"6eb6854\" data-element_type=\"widget\" data-widget_type=\"spacer.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<div class=\"elementor-spacer\">\n\t\t\t<div class=\"elementor-spacer-inner\"><\/div>\n\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-5247b73 elementor-widget elementor-widget-heading\" data-id=\"5247b73\" data-element_type=\"widget\" id=\"subtopic09\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Anti-phospholipid syndrome<\/h3>\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-327b721 elementor-widget elementor-widget-text-editor\" data-id=\"327b721\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<p>A syndrome characterized by \u201csticky blood\u201d was described by Dr Graham Hughes in 1983, and is associated with the presence of circulating antibodies against phospholipids. Two tests are usually requested \u2013 anti-cardiolipin antibody and lupus anti-coagulant. The major features of anti-phospholipid syndrome are:<br \/>\u00a0<\/p><ul><li>vein thrombosis, i.e. Arm vein thrombosis, thrombosis in the eye, leg vein thrombosis, and thrombosis of the veins of the brain.<br \/>\u00a0<\/li><li>arterial thrombosis, i.e. Leg artery clots or heart attack.<br \/>\u00a0<\/li><li>recurrent miscarriage<\/li><\/ul><p><br \/><br \/>Other features are low platelet count, blotchy skin rash, migraine, and positive blood tests for anti-phospholipid antibodies.<br \/><br \/>For patients with anti-phospholipid syndrome, pregnancy loss is most typically in the second trimester. Treatment is with aspirin plus heparin. Heparin is stopped at the time the patient goes into labor but is resumed after delivery. The greatest thrombotic risk to mother is during the post-partum period.<br \/><br \/><\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-c7331b4 elementor-widget elementor-widget-spacer\" data-id=\"c7331b4\" data-element_type=\"widget\" data-widget_type=\"spacer.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<div class=\"elementor-spacer\">\n\t\t\t<div class=\"elementor-spacer-inner\"><\/div>\n\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-445eb16 elementor-widget elementor-widget-heading\" data-id=\"445eb16\" data-element_type=\"widget\" id=\"subtopic10\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Childhood<\/h3>\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-4642138 elementor-widget elementor-widget-text-editor\" data-id=\"4642138\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<p>Although there are few long-term follow-up studies of children born to SLE mothers, there seem to be very little risk of SLE in the children born to patients.
